Paragraph 1 outlines what happened and when it happened.
Paragraph 2 to 4 outlines gives more detail over the incident. Remember, please start a new paragraph for every new idea.
Paragraph 5 is for you to give your suggestions and opinions..
Please remember your paragraphing, and there is no need to sign off. Just leave it as it is...
